1간호간병 통합서비스 의사와 간호사 대상 의사소통 향상 교육프로그램의 개발 및 적용 효과

목적: 이 연구의 목적은 간호간병 통합서비스에서 근무하는 의사와 간호사들을 대상으로 의사소통 향상 교육 프로그램을 개발하고 적용하여 효과를 검정하는 것이다. 연구방법: 의사소통 향상 교육프로그램 개발을 위해 의사와 간호사를 대상으로 포커스 그룹 인터뷰를 통하여 요구도를 조사하고 문헌고찰과 전문가 집단의 협의를 통하여 교육 프로그램을 구성하였다. 프로그램 효과를 검정하기 위해 간호직종은 비동등성 대조군 전후 설계를 이용하여 의사소통 교육 프로그램을 제공한 군과 제공하지 않은 군 간의 비교를 하고, 의사직종은 교육 제공 전, 후 사전사후 조사를 실시한 단일군 전후 설계를 이용하였다. 의사소통 향상 교육프로그램은 2017년 4월부터 8월까지 4개월 동안 3회의 중재 및 자료수집이 이루어졌으며, 연구 대상은 간호간병 통합서비스 병동에서 일하는 의료진 가운데 연구에 동의한 의사 23명, 간호사 109명이었다. 자료 분석은 SPSS 19.0을 이용하였다. 연구결과: 의사소통 향상 교육프로그램을 적용하기 전ㆍ후 의사소통 능력 점수는 의사직종이 77점에서 84점으로 유의하게 증가하였고(Z=-2.10, p=.036), 간호직종에서도 84.67±7.45점에서 88.46±7.45점으로 유의하게 증가하였다(t=3.44, p=.001). 직무스트레스 영역 가운데 관계갈등 부분에서 의사직종은 66.67점에서 58.30으로 유의하게 감소함을 보여주었다 (Z=-2.34, p=.019). 결론: 간호간병 통합서비스에서 일하는 의사와 간호사를 대상으로 수행한 의사소통 향상 교육프로그램은 의사소통 능력을 높이고, 직무 스트레스에서 관계갈등을 감소시키는 효과가 있었음을 알 수 있었다. 향후 연구에서는 의사소통 교육 프로그램을 증대하여 운영 및 지속 적용 운영 후 효과를 검증하는 연구를 제언한다.


Purpose: The purpose of this study was to develop Communication Improvement Education Program and identify effects of the program for nurses and physicians working in hospital. Methods: This study was a nonequivalent control group pretest-posttest design on 109 nurses and one group pretest-posttest design on 23physicians. Data were collected from April 2017 to August 2017. A Communication Improvement Education Program was applied on 54nurses and 23physicians three times for 4months. The collected data were analyzed using SPSS 19.0. Results: The physician group showed increasing communication ability score and decreasing relationship conflict after Communication Improvement Education Program completion and had a statistically significant(p=.036). The experimental group showed higher score(88.46±7.45) in communication ability compared to the control group(83.51±7.58) and showed a statistically significant difference(p=.001). Conclusion: The Communication Improvement Education Program was proved effectiveness in improving communication ability for nurses and physicians working in Nursing care Integrated Service ward. Further research on the application Communication Improvement Education Program over 3times that can improve communication skill and relationship conflict score and that program is essential for nurses and physicians.

Purpose: The purpose of this qualitative study was to describe the essence and the meaning of nurses’ experiences of end-of-life care (EOLC) for elderly patients in the long-term care hospitals (LTCHs). Methods: Data were gathered from 12 nurses who had been working at LTCHs in Korea through one-on-one interviews and the data were analyzed by Colaizzi's phenomenological method. Results: The emergent 5 themes were ‘Doing the best for protecting patients’ life’, ‘Providing a comfortable dying process for patients’, ‘Supporting a family’s keeping on patient’s death’, ‘Reflecting on life’ and ‘Desiring for the establishment of a humanity end-of-life care environment’. Conclusion: The end-of-life care for the elderly patients includes supporting elderly patients’ comfortable dying process and helping the family keep the death of the elderly. The results indicated that physical facilities and end-of-life nursing capacity should be established in LTCHs for improving the quality of EOLC.

Purpose: The purpose of this study was to compare the moral distress and burnout between long-term care hospital nurses and general nurses. Methods: This study was a cross-sectional survey. Participants were 193 nurses (long-term care hospital 95, general hospital 98) working in a hospital in Seoul, Daegu and Busan city. Data were collected from October 02 to November 30, 2018 using a structured questionnaires and analyzed with SPSS/PC ver 21.0 programs. Results: The scores of moral distress by nurses working in long-term care hospitals were higher than those of nurses in general hospitals. The score of burnout by nurses working in general hospitals was significantly higher than that of nurses in long-term care hospitals. There was a significant positive relationship between moral distress and burnout in both long-term care hospital nurses and general hospital nurses. Conclusion: This research shows that long-term care hospital nurses have higher moral distress and burnout than those of general hospital nurses. Therefore, nursing managers and organization should strive to raise the perception of moral distress and burnout in long-term care hospital nurses.

Purpose: This study aimed to explore the difficulties and coping that advanced practice nurses (APNs) experience in home health nursing field. Methods: The participants were 12 APNs who had experience in home health nursing for over 5 years. Data were collected through two focus group interviews between August and September, 2017. The results were analyzed using a qualitative content analysis. Results: Out of 10 sub-categories and 29 codes, 4 categories emerged: “crushed by the responsibilities as the only healthcare professional”, “worn out by family who interfere with work”, “anxiety about my safety being threatened” and “using self-learned know-hows”. Conclusion: APNs face difficulties due to the environmental characteristics of the homes, the characteristics of chronically critically ill (CCI) patients and their families, and legal or systematical limitations when dealing with problems based on their personal competence. In order to ensure that professional nursing care is more readily provided to CCI patients through home health nursing services in local communities, it is imperative that support systems at the organizational and institutional levels be established in a systematic way to resolve the issues faced by APNs.

Purpose: The aim of this study was to provide information on the extent and variations of elderly residents’ nursing care needs, and provision of nursing care across long term care facilities. Methods: A nationwide survey was conducted on nurse managers from 1,041 long term care facilities, by e-mail or fax, from August 16 to September 30 in 2017. A self-reported questionnaire consisting of 5 domains was used to collect data. Results: Facilities with more than 30 residents were more likely to need skilled nursing services and to obtain the nursing staff such as a registered nurse and a nurse’s aide. Awareness and satisfaction of hospital-based home nursing care was high in all facilities. In addition, there are some differences in nurse managers’ perceptions of the level of healthcare resources and required action by facility size. Nurse managers of senior congregate housings were more likely to have considerable difficulty in dealing with healthcare needs of residents and recognizing the healthcare resource shortage. A majority of nurse managers agreed on the need to employ a registered nurse. Conclusion: This study confirmed that it is essential to increase nurse staffing level and to reform the long term care insurance for enhancing the accessibility of healthcare services, especially for the residents in small long term care facilities. There is also a need to provide diverse education and training opportunities for nursing staff working in long term care facilities.

Purpose: The purpose of this study was to investigate the factors that affect the quality of nursing services, as perceived by musculoskeletal surgical patients who are hospitalized in comprehensive nursing care wards. Methods: A cross-sectional study was conducted, with 152 musculoskeletal surgical patients hospitalized of I hospital and K hospital of B City in South Korea. The data were collected from July 26 to October 15, 2019. Analysis of the data used real numbers, percentages, means and standard deviation, minimums, maximums, mean ratings and standard deviations, a t-test, ANOVA and the Scheffé test, Pearson’s Correlation Coefficients, and Stepwise Multiple Regression using the SPSS/WIN 23.0 statistical program. Results: The factors that had the greatest effect on patients’ perceived quality of nursing services were nurses’ friendly communication types (β=.25, p=.012), followed by education level (β=.49, p<.001), and the level of satisfaction with nurses’ pain management performance (β=.39, p=.001). The total explanatory power was 30.4%. Conclusion: In order to improve the quality of nursing services as perceived by patients, it is necessary to provide customized nursing interventions taking patients’ education levels into consideration and to develop programs to improve pain management and friendly communication by nurses.

7요양병원간호사의 죽음인식, 임종간호스트레스와 임종간호수행간의 관계 조사

본 연구는 요양병원에 근무하는 간호사를 대상으로 죽음인식, 임종간호스트레스, 임종간호수행간의 관계를 파악하고 이를 기반으로 임종간호수행에 영향을 주는 주요 요인을 파악하기 위한 것이 궁극적인 목적이다. 본 연구는 전라남도 3개 도시에 위치한 요양병원간호사를 대상으로 연구를 수행하였으며, 조사도구는 죽음인식, 임종간호스트레스, 임종간호수행 설문지를 활용하였다. 수집된 자료는 기술통계를 수행하였으며, 변수간의 상관성을 분석하기 위하여 피어슨상관분석을 수행하였으며, 주요 영향인자를 분석하기 위한 회귀분석을 실시하였다. 통계분석은 SAS 9.1 ver.을 사용하였다. 간호사 자신의 정신적, 업무적 스트레스가 임종간호수행에 많은 어려움을 야기하고 간호사는 임종환자를 자주 경험하면서 임종간호스트레스가 높아진다. 죽음인식과 임종간호스트레스는 임종간호수행과 상관관계가 있는 것으로 나타났으며, 임종간호수행에 영향을 미치는 요인은 임종간호스트레스와 임상경력 이었다. 요양병원간호사의 임상경력이 많은 사람일수록 임종간호수행도가 높았으므로 임상경력을 고려한 임종간호수행이 이루어져야 하며, 임종간호수행에 있어 스트레스를 줄일 수 있는 방안과 임종간호수행에 영향을 주는 다른 요인을 심도 있게 파악하여 요양병원 실정에 맞는 임종간호수행 및 교육이 필요하다고 사료된다.


The purpose of this study is to investigate the relationships between death recognition, terminal care stress, terminal care performance, and other influencing factors of terminal care performance in terminal care hospital nurses. Two hundred forty nurses working in 11 hospitals for the elderly located in G, N, C city were surveyed. They were asked to complete three questionnaires: one on death recognition, one on terminal care stress, and a final one on terminal care performance. The data collected were analyzed using descriptive statistics, Pearson's correlation, and stepwise regression using SAS 9.1. The findings suggest that the nurses' own mental and occupational stress was the cause of many difficulties in the practice of end-of-life care, where the nurse will often experience the death patients, which results in increased dysentery stress. There was a negative correlation between species stress and terminal care performance. Death recognition and terminal care stress were correlated with terminal care performance. The results suggest that the longer the clinical experience, the higher the quality of terminal care performance. Therefore, it is suggested that terminal care should be performed considering the longevity of the careers career and their death tendency. Further to this study, it is necessary to find out how to reduce stress and to evaluate other variables affecting the care of the patient.

Purpose: The purpose of this study was to explore the experiences of nursing students’ practice on community visiting nursing. Methods: For this study, in-depth interviews were conducted with a total of 12 nursing students, and data were collected through individual in depth interviews from September to December 2018. The data were analyzed using the phenomenological analysis method suggested by Colaizzi. Results: The experience of visiting nursing practice consisted of four themes. Four themes are ‘Understanding visiting nursing work: Tight time and problems to overcome’, ‘Understanding visiting nursing object: People waiting for a visiting nurse’, ‘Visiting nursing practice experience: Sometimes to get reprimanded but rewarding practice’, and ‘Expectations for visiting nursing business: Hopes for development’. Conclusion: Findings from this study presented vivid experiences of nursing students who on community home visit nursing. Community visiting nursing practice became an opportunity for students to understand visiting nursing and target people and to think about the necessity of visiting nursing and future development direction. It is necessary to develop an institutional and policy basis with regard to the increasing number and role expansion of community visit nurses.

Purpose: The purpose of this study was to examine the effects of the leader-member exchange and organizational commitment on their nursing work performance of clinical nurses. Methods: This study participants included 213 nurses working at a tertiary hospital in B city in south Korea. Data were collected by self-reported questionnaires and included leader-member exchange, organizational commitment and nursing work performance. The data were analyzed using descriptive statistics, independent t-test, one way ANOVA, Pearson’s correlation coefficients and multiple regression analyses with IBM SPSS 18.0 program. Results: The significant predictors of nursing work performance were organizational commitment (β=.25, p<.001), clinical career (β=.21, p=.002), relationship with colleagues (β=.20, p=.001), contribution as a sub-area of leader-member exchange (β=.17, p=.005), clinical career in current department (β=.13, p=.049), and religion (β=.12 p=.031). These variables explained 33.2% of the variance in nursing performance. Conclusion: It is suggested to highlight the impotance of nursing work performance and to develop and execute the nursing intervention program for enhancing leader-member exchange relationships and organizational commitment on the basis of appropriate career management for nurses and the enhancement of relationships with colleagues.

Purpose: The purpose of this study was to identify levels of health literacy and examine the effects of health literacy and self-care activities on quality of life of patients with type 2 diabetes. Methods: A cross sectional design was used. Questionnaires were administered via the Gallup to 500 Korean patients with diabetes in Seoul and Gyeonggi areas. The data were analyzed using descriptive statistics, independent t tests, one way ANOVA, and hierarchical regression. Results: Of 500 patients, 12.4% (n=62) indicated ‘inadequate’ health literacy, and 43.8% (n=219) reported ‘marginal’ or ‘adequate’ health literacy. Levels of health literacy significantly differed by demographic characteristics, self-care activities, and quality of life. The factors that had a significant effect on quality of life included inadequate health literacy (B=3.63, p=.042) and marginal health literacy (B=3.84, p=.002); however, no significant relationship was found for self-care activities (B=0.01, p=.827). Conclusion: The results of this study demonstrate a need for further research with large samples using Korean Health Literacy Scale for Diabetes Mellitus to establish the relationships between health literacy, diabetes self-care activities, and quality of life.
