1`군진간호연구` 게재논문의 연구동향 분석(2011-2015)

Purpose : This study is to analyze the trends of research that was published in the Korean Journal of Military Nursing Research from 2011 to 2015. Methods : Analyzing a total of 91 studies in the categories of research types, methods, and key words. Descriptive statistics were used. Results : 56% of the studies were funded, and 7.7% used a theoretical framework. An average study included 3.3 authors, and 38.5% were military-civilian joint projects. Quantitative research (79.1%) was most frequently conducted, followed by qualitative (11.0%) and literature review (5.5%). Less than 40% reported that they were approved by an IRB. An average of 3.3 instruments were used in a study, and 49.1% were psycho-social ones. Among others, inferential statistics were preferred in data analysis (69.7%) with the highest use of t-test (32.9%). The health category among 4 meta-paradigms of nursing involved 39.1% of the key words. Conclusion : The Korean Journal of Military Nursing Research is one and only journal devoted to military nursing in the country. Therefore, more military-specific and future-oriented projects should be conducted as a way of maintaining its distinct characteristics and improving military nursing practice, education, and research.

Purpose : This study examines the mediating effects of self-directed learning among the influence of career value and career planning on academic achievement and career adaptability of the cadets from Korea Armed Forces Nursing Academy. Methods : The subjects of this study are 321 students of all grades attending Korea Armed Forces Nursing Academy. The collected data is analyzed by SPSS win 20.0 program. Results: First, the relationship between career value and academic achievement is not significant, and the full mediating effect of self-directed learning is proved in relation to career planning and academic achievement. Second, in career adaptability, both career value(Intrinsic·Extrinsic) and career plan show a static relationship. Meanwhile, self-directed learning is found to fully mediate intrinsic career value, while partially mediating extrinsic career value and career planning. Conclusion : The results of this study suggests ways to improve the academic achievement and the level of career adaptation of the ROK military nurses. That is, in addition to the wide and deep value education that can be harmonized with the self of the individual, there is a need to establish well-organized education system that equips them for diverse career experiences after being commissioned. Cadets should also learn the concepts and methods of self-directed learning through autonomous participation in the course of cadet education, calling for the education system that can restore emotional self-esteem.

Purpose : This study is to investigate the overall use of standardized nursing statements in NDEMIS, the electronic nursing record system of Korean military hospitals and to identify frequently used statements among them by hospitals and treatment departments. Methods : The authors analyzed the nursing records of randomly chosen 4 weeks from 12 military hospitals in 2015. Descriptive statistics were used to investigate the utilization rate of nursing problems, nursing interventions, and nursing activities in the standardized nursing statements. Results : Analyzed were 245,252 nursing records of 9,532 patients who had been admitted to military hospitals. The nursing records contained 48.0% of the standardized nursing statements. Among them, the most frequently used statements were `acute pain` for nursing diagnosis, `Pain management` for nursing intervention, `notify medical team if pain worsens` and `assess pain` for nursing activity. The pattern of standardized nursing statements` use by hospitals and treatment departments reflected general characteristics of patients admitted to the military hospitals. Conclusion : This study indicates that the priority in military nursing care is management of pain. Considering the percentage of standardized nursing statements, education for nursing staff should be regularly provided to promote use of them. Also NDEMIS should be improved according to the user`s needs so that the system can be fully utilized.

Purpose : The aims of this study are to identify the relations between military nursing staff`s perception and satisfaction on communication with other nursing staff and physicians and their job satisfaction in the armed forces hospitals in Republic of Korea. Methods : A cross-sectional descriptive survey design was used. Data were collected from 163 nurse officers and civilian nurses working in five Korean military hospitals in June, 2017. Used were ICU Nurse-Physician Questionnaire to measure their perception on communication with other nursing staff and physicians, two 5 Likert scale questions of communication satisfaction, and Index of Work Satisfaction. Results : Nursing staff`s perception on communication with other healthcare professionals were significantly different by their work units (p<.001). Their satisfaction rate on communication presented significant differences by level of education (p=.021) and work unit (p<.001). Likewise perception on communication, nursing staff`s job satisfaction rate demonstrated significant differences among different work place (p<.001). Nursing staff`s perception and satisfaction on communication and their job satisfaction had significantly positive correlations (p<.001). Conclusion : In order to improve military nursing staff`s perception and satisfaction on communication with other healthcare professionals, it is necessary to develop a program to promote better communication among healthcare professionals.

Purpose : The purpose of this study is to develop an integrated nursing curriculum on patient safety for the 4th grade cadets of Korea Armed Forces Nursing Academy. Method : For the Integrated nursing curriculum development, Dick & Carey`s ADDIE model was used. The analysis, design, and development phases were conducted in this study with the exception of implementation and evaluation. In the analysis stage, patient safety guidelines and the education needs of nursing officers in 10 military hospitals were analyzed. Based on the analysis, interviews, and literature review, the authors drafted education topics and contents in the design stage. During the development stage, the validity of the curriculum was checked by experts. Result : Education in four major categories of medication errors, falls prevention, communication, and nursing practice were in high demand. Each category was comprised of its own content items, and finally 39 items in total passed the validity test. The validity score of the 39 items ranged form 0.8 to 1.0. Based on these results, a 30-hour or 4-days program focusing on patient safety was developed integrating adult nursing, fundamental nursing, psychiatric nursing, and nursing management. Conclusion : This study is expected to contribute to improving clinical competency of newly commissioned nursing officers especially for patient safety. Further research needs to determine the effect of the integrated nursing curriculum.

6청장년층 뇌졸중 환자의 Prehospital delay와 영향요인

Purpose : This study examines stroke patients` prehospital delay and its influencing factors to provide basic information for effective management and treatment of young adult and middle-aged stroke patients. Methods : The analyzed medical records were 260 stroke patients who were admitted to a university hospital from January, 2012 to April, 2015. The data included the time range from symptom outbreak to arrival to the hospital, National Institute of Health Stroke Scale (NIHSS) score, and general characteristics. Descriptive statistics, chi-square test, paired t-test, and multinomial logistic regression were conducted. Results : Only 33.1% of the patients arrived at the hospital within 3 hours while 55.8% of them took 6 hours or longer after stroke symptoms` onset. Those who arrived within 3 hours after the symptom onset demonstrated a significant decrease in NIHSS scores when they were discharged, compared to those measured upon arrival, by 1.36 for those in their 40s (p=.010) and 1.04 in their 50s (p=.015), respectively. Sex, age, medical history of atrial fibrillation, and type of stroke were found to influence on prehospital delay of 6 hours or longer. Conclusion : Although prehospital delay affects the prognosis of stroke patients, the number of patients entering a medical facility within 3 hours after onset of symptoms is still low. This study suggests a longitudinal future research to identify influencing factors on prehospital delay by different age groups and to develop education programs for each group.

Purpose : The purpose of this study was to examine the effect of Vitamin C supplementation and walking exercise on depression and empowerment among middle-aged women. Methods : The study design was single group pre-post experimental design. Participants were 30 middle-aged women aged 40-57 years. Participants took vitamin C supplement and performed walking exercise for 4-8 weeks. Data were collected by the two kinds of self-reported questionnaire (Beck Depression Inventory, Intrapersonal Empowerment Scale). Data were analyzed using descriptive statistics, paired t-test and ANOVA. Result : After the vitamin C supplementation and walking exercise, participants showed significant decreases in depression(t=6.09, p<.001) and significant increases in empowerment (t=2.35, p=.027) Conclusion : Result shows that vitamin C supplementation and walking exercise are effective in decreasing depression and increasing empowerment of middle-aged women.

Purpose : The purpose of this study was to identify influence of emotional intelligence and critical thinking disposition on self-leadership in nursing officers in armed forces hospitals. Methods : Initial data were collected from October, 11th to 28th, 2016 with self-report questionnaires at three armed forces hospitals in Korea. 175 subjects agreed to participate in this study. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, and stepwise multiple regression using the SPSS 21.0 program. Results : This study showed that emotional intelligence and critical thinking disposition are positively correlated with self-leadership, and critical thinking disposition, emotional intelligence, and education level explained 41.9% of self-leadership. Conclusion : The results confirm that nursing officers` emotional intelligence and critical thinking affects self-leadership. Accordingly, to improve nursing officers` self-leadership, it is necessary to develop and apply education program that can improve self-leadership as well as emotional intelligence and critical thinking disposition.

Purpose : The purpose of this correlational survey is to investigate junior nursing officers` confidence and performance frequency of core basic nursing skills and their clinical competency. Methods : The data were collected from 208 junior nursing officers with structured questionnaires between April 12 and May 8. The data were analyzed with descriptive statistics, t-test, ANOVA, and Pearson`s correlation coefficient using SPSS. Results : Confidence and performance frequency of core basic nursing skills and clinical competency of the participants were 4.16±.69, 2.56±.57, and 4.32±.44 respectively. There was a significant difference in performance frequency of core basic nursing skills depending on years since commission and place of work. And there was a significant difference in clinical competency depending on specialty. There were statistically positive correlations among junior officers` confidence and performance frequency of core basic nursing skills and clinical competency. Conclusion : The study results suggest that it is necessary to develop stepwise education strategies for clinical practice including core basic nursing skills in consideration of place of work and roles of junior nursing officers.
