1성인기와 노년기별 암 경험자의 유병기간이 우울감 변화에 미치는 영향

Purpose: The purpose of this study is to identify the effect of the disease duration on depression change in cancer survivors between the middle and old age groups.
Methodology: To that end, we analyzed 275 patients using the Korea welfare panel survey from 2011 to 2016 jointly developed by the Korea Institute for Health and Social Affairs and Seoul National University. The duration of the elapsed and the experienced was measured as a independent variable. CES-D was used as a dependent variable. Frequency, paired t-test, ANOVA, and logistic regression analysis were conducted using Stata 14.0 for statistical analysis.
Findings: Studies have shown that as the duration of the elapsed and the experienced increases, people under 65 are 1.17 times and 1.84 times each more likely to be included in depression-increase group than in depression-reduction group. The group that has been maintaining economic activities continuously since 2011 was significantly less likely to belong to the depression-increase group than the group that did not have economic activities in 2016. For those aged 65 and older, there was no significant difference in the duration of the elapsed and the experienced. The higher the likelihood of low-income households being included in the family of depression was 2.58 times higher than for ordinary households.
Practical Implications: It is suggested that close policy management of the employment and working environment of cancer survivors is necessary, as both cancer survivors of productive age as well as older cancer survivors are analyzed to be beneficial to depression management. In addition, a systematic management program related to the mental health of cancer survivors is believed to be necessary for normal social recovery in the future.

Purposes: The purpose of this study is to analyze the factor of affecting on job satisfaction by mediating attitude of care service.
Methodology: (1)Identify socio-demographic characteristics, work characteristics, the elderly support period, the presence of education related to the elderly, the presence of service associated with the elderly, and significant factors of the research subjects. (2)Identify changes in the number of education and services before and after obtaining a caregiver qualification. (3)Identify the attitude of providing care services and job satisfaction according to the characteristics of the research subjects. (4)Identify the direct and indirect factors influencing job satisfaction by mediating the service attitudes of the research subjects.
Findings: First, age factors have positive effects and the volunteering experience have negative effects on job satisfaction by mediating attitude of care service. Although the number of education class has increased after qualification, educational factors had no significant effect. Second, the longer elderly support period had a positive effect on job satisfaction.
Practical Implications: This study is meaningful in that it identifies the direct and indirect impacts on job satisfaction through care attitudes. In the future, it will be necessary to pay attention to improving the quality of long-term care services by analyzing the influence factors more.

Purposes: This study was to analyze the effects of organizational culture attributes on organizational effectiveness in newly established general hospital.
Method: For this purpose, this study sampled 981 hospital employees working for E hospital opened on Apr. 1, 2019 in Seoul. A total of 981 questionnaires were distributed to them, and 888 ones responded to the survey, which had been conducted from Oct. 17, through Oct. 25, 2019. 793 responses were used for the final analysis. The data collected were processed using the SPSS 19.0K for descriptive statistics, T-test, ANOVA, Pearson's correlation coefficient and regression analysis.
Findings: First, The type of organizational culture perceived most by hospital employees was ʻhierarchy-orientedʼ (3.53) followed by ʻrelation-orientedʼ(3.33), ʻtask-orientedʼ(3.23), ʻinnovation-orientedʼ(3.19) and job satisfaction scored 3.13, organizational commitment scored 3.28 on their order. Second, ʻRelation-orientedʼ, ʻtask-orientedʼ and ʻinnovation-orientedʼ was positively correlated with job satisfaction and organizational commitment. hierarchy-orientedʼ was negatively correlated with job satisfaction and organizational commitment. Third, Factors that significantly influencing on organizational effectiveness in a newly established general hospital were as follows. Factors influencing job satisfaction included ʻinnovation-orientedʼ, ʻrelation-orientedʼ and factors influencing organizational commitment included ʻrelation-orientedʼ, ʻinnovation-orientedʼ and age.
Practical Implications: In order to increase the organizational effectiveness of the E-General Hospital, it is necessary to gradually move from a hierarchica l-oriented culture to an innovation-oriented and relationship-oriented culture. Since age also affects organizational commitment, it is necessary to promote stability and growth by promoting the mentor-menti system for new members with a low level of experience and experience.

Purpose: The purpose of this study is to investigate medical care behaviors influencing accuracy of the payment based New diagnosis-related groups (DRG) compared to fee for service (FFS) in hospitalized patients with medical illness.
Methodology: In order to estimate the difference in medical costs between New DRG and FFS depending on medical care behaviors, medical records and hospital claims data (n=4,232) were utilized, which were collected from a single public hospital during the first-half of 2018. Data were analyzed by descriptive statistics, t-test, chi-square test, and multivariate binary logistic regression.
Findings: The average difference in medical costs between New DRG and FFS were KRW 506,711±13,945 with incentives and KRW -51,506±12,979 without incentives, respectively. Forty-four point two percent (44.2%, n=1,872) of total subjects were shown to have negative compensation in overall medical costs with New DRG compared to the costs with FFS. Medical care behaviors that affected on the negative compensation were the presence of severe bed sores on admission, medical consultations, death, operations, medications and laboratory or imaging tests with unit price over KRW 100,000, hospital-acquired complications or underlying comorbidities, elderly patients (≧65 years), and hospitalized for more than average inpatient days defined by New DRG (p<0.001). The difference in average medical cost between New DRG and FFS for a group with mild illness was KRW -11,900±10,544, whereas it was KRW -196,800±46,364 for a group with severe illness (p<0.0001).
Practical Implications: These findings suggest that New DRG payment model without incentives may incompletely cover the variation of medical costs in real clinical practice. Therefore, policy makers need to consider that the current New DRG reimbursement should be focused and refined to improve accuracy of payment on medical care resources utilized in severe and complex medical conditions.

Purpose: The purpose of this study is to determine the influence of hospital employees' perceptions and performance expectations on IT technologies introduction and intention to use in the Fourth Industrial Revolution.
Methodology: A survey was conducted on 524 employees at 42 hospitals. Pearson's correlation analysis, regression analysis, Sobel test, and structural equation were used for the analysis.
Findings: The factors of technical situation (complexity, compatibility, technical readiness, and security concerns), those of environmental situation (competitive pressure and regulatory support), and organizational situation (top management support and costs) have influenced on the recognition of employees in the hospital and the performance expectations. IT technology introduction and use intention have been influenced by technical situation, organizational situation, and environmental situation in order. But security concerns and cost factors have not statistically affected. In addition, due to the perception and performance expectations of hospital employee who have a role in mediating the introduction and use intention of IT technology, the greater the perception and performance expectations of hospital employees, the higher the intention of introduction and usage of IT technology.
Practical Implications: Rather than considering the position of external customers and the position of competitive hospitals, more attention is required for the perception and performance expectations of the internal members.

Purposes: This study aims to investigate the number of cataract surgeries and predict future trends using 13-year data.
Methodology: Trends investigation and comparison of prediction methods was conducted to determine better prediction model using Major Surgery Statistics from Korean Statistical Information Service in 2006-2018. ARIMA(Auto Regressive Integrated Moving Average) was selected and prediction was conducted using R program.
Findings: As a results, the number of surgeries will continue to increase. The trends was predicted to increase during January-April, and it declined over time and was the lowest in August.
Pratical Implications: Therefore, it is necessary that management will be needed by continuously investigating and predicting the demand and trend for surgery to prepare an alternative to the increase.
