1간호사 괴롭힘 관련 인터넷 포털 기사에 대한 댓글의 의미연결망 분석

Purpose: A significant amount of public opinion about nurse bullying is expressed on the internet. The purpose of this study was to analyze the linkage structures among words extracted from comments on internet articles related to nurse workplace bullying using semantic network analysis. Methods: From February 2018 to April 2019, comments made on news articles posted to the Daum and Naver web portal containing keywords such as “nurse”, “Taeum”, and “bullying” were collected using a web crawler written in Python. A morphological analysis performed with Open Korean Text in KoNLPy generated 54 major nodes. The frequencies, eigenvector centralities, and betweenness centralities of the 54 nodes were calculated and semantic networks were visualized using the UCINET and NetDraw programs. Convergence of iterated correlations (CONCOR) analysis was performed to identify structural equivalence. Results: This paper presents results about March 2018 and January 2019 because these months had highest number of articles. Of the 54 major nodes, “nurse”, “hospital”, “patient”, and “physician” were the most frequent and had the highest eigenvector and betweenness centralities. The CONCOR analysis identified work environment, nurse, gender, and military clusters. Conclusion: This study structurally explored public opinion about nurse bullying through semantic network analysis. It is suggested that various studies on nursing phenomena will be conducted using social network analysis.

2국내 임상간호사의 긍정심리자본 관련 요인: 체계적 문헌고찰 및 메타분석

Purpose: The purpose of this study was to systematically review and identify factors relevant to the positive psychological capital of clinical nurses. Methods: These was no limit on year of publication. Articles related to Korean clinical nurses were retrieved from computerized database using a manual search. A systematic review was conducted based on the PRISMA flow. The total correlational effect size (ESr) for each related factor was calculated from Fisher's Zr. Funnel plots, fail-safe numbers, and Egger regression tests were used to evaluate publication bias in meta-analysis studies. The correlational effect size of 25 studies was analyzed through meta-analysis using Comprehensive Meta-Analysis software 3.0 (CMA). Results: The review included 25 studies. In the systematic review, 14 demographic factors and 46 organizational factors were found to be influential. Eleven factors (6 demographic factors and 5 organizational factors) were appropriate for meta-analysis. The overall effect size was .26. The demographic total correlation effect size of related factors was .20 and the total effect size of organization was .46. Organizational commitment (ESr=.38) and job satisfaction (ESr=.54) were statistically positively related variables. Negative variables were burnout (ESr=-.61), turnover intention (ESr=-.41) and workplace bullying (ESr=-.33). The total effect size of the organizational factors was larger than the demographic total effect size. There was no publication bias except for demographic variables. Conclusion: Organizational factors and adjustable variables have a significant impact on positive psychological capital. The results of this study support the need for development of interventions focusing on organizational factors.

Purpose: The aim of this study was to investigate the satisfaction of newly graduated nurses with educational programs and their experiences in role transition. Methods: Data were collected from November 1 to December 15, 2018 and 483 new graduate nurses working at 15 tertiary hospitals and 10 general hospitals participated. For data collection, self-report questionnaires including the Casey-Fink Graduate Nurse Experience Survey tool and satisfaction with education were used. Data were analyzed using descriptive statistics, t-test, and one-way analysis of variance. Results: Satisfaction with education ranged from 3.09 to 3.27, and satisfaction with preceptors was 3.45(maximum 4). The skill that new nurses ranked as most difficult during the first 3 months was charting/documentation, and throughout a whole year, the top 4 difficult skills were cardiopulmonary resuscitation/emergency response, ventilator care, end-of-life care, and prioritization/time management. In comfort/confidence, new graduates felt most comfortable with support and least comfortable with patient safety. More than 50 percent of new graduates experienced stress during role transition, and the most frequently experienced stressors were related to job performance and personal life. Levels of satisfaction with education and comfort/confidence differed according to the hospital type and number of preceptors for new nurses. Conclusion: In order to facilitate the transition of new graduate nurses to professional nurse, an extended period of education, systematic and standardized transition programs, and continuous support during the first year of practice are required.

Purpose: This study was a systematic review and meta-analysis to explore the factors related to quality of life in heart transplant recipients. Methods: To identify studies that suggested the factors related to the quality of life in heart transplant recipients, we searched the articles published from 1974 to November 2018 using Six databases, PubMed, CINAHL, EMBASE, Cochrane, KMBASE and RISS. A total of 22 studies were selected out of 5,234 for the systematic review and meta-analysis on the basis of the PRISMA flow. The quality of study was assessed by assessment tool form the NIH and meta-analysis was performed using the 'R 3.5.2' version to analyze the correlated effect sizes. Results: Factors related to quality of life in heart transplant recipients were categorized into six domains based on the health-related quality of life model introduced by Ferrans: individual, environmental, biological function, symptoms, functional status, and general health perception. In the meta-analysis, 34 factors were used and 17 factors having significant effect sizes were as follows: self-efficacy, demoralization, perceived control, current occupational status, age, marital status, health promotion life style in the individual characteristics; stress in environmental characteristics; physical function status, creatinine level, left ventricular ejection fraction (LVEF) in biological function; anxiety, depression, symptom frequency and distress in symptoms domain; coping, self-care compliance in functional status. Conclusion: The findings indicate that the multi-dimensional factors influencing the quality of life in heart transplant recipients and provide the evidence for developing effective interventions for improving the quality of life of recipients.

Purpose: The purpose of this study was to identify the effects of psychological acceptance and social support on posttraumatic growth in stomach cancer patients. Methods: The questionnaires were administered from January 14 to February 11, 2015 to 123 subjects who had stomach cancer surgery six months prior. SPSS statistics 21.0 software was used to analyze the data for t-test, ANOVA, Pearson's correlations, Scheffe test and multiple regression analysis. Results: The results of this study are as follows: The major factors related to posttraumatic growth included gender (t=-2.72, p=.007), age (r=-.21, p=.016), having a religion (t=-3.40, p<.001), perceived importance of religion (r=.43, p<.001), seriousness of cancer diagnosis (r=.25, p=.005) and impact of cancer diagnosis (r=.32, p<.001). There were significant relationships between psychological acceptance (r=.18, p=.041) and social support (r=.32, p<.001) on posttraumatic growth. Significantly influential factors of posttraumatic growth were age (β=-.19, p=.021), perceived importance of religion (β=.41, p<.001) and family support (β=.29, p<.001), which together accounted for 36.5% of the variance in posttraumatic growth. Conclusion: The result of current study indicated that age, importance of religion, and family support influenced posttraumatic growth. Based on the findings of this study, developing nursing intervention programs focusing on increasing posttraumatic growth in stomach cancer patients is recommended.

Purpose: The purpose of this study was to analyze the current state of home health nursing (HHN) for elders and to provide basic data on policy alternatives for establishing home medical care in the advanced general hospital. Methods: This study was conducted as a secondary data analysis, using electronic medical record (EMR) data of older patients who received HHN more than once from the S advanced general hospital between January 2016 and December 2018. Results: A total of 1,790 patients received HHN visits, with 22,477 visits being made. The mean age was 76.8±7.3 years old, 96.0% of elders had health insurance and 24.6% had orthopedics problems. Of the 1,168 people who visited emergency rooms, the most frequent symptom was pain (23.4%) and all patients visited the hospital at least once and at most 163 times outpatient care during HHN. Causative diseases were degenerative knee joint osteoarthritis (0.6%), surgery for right knee replacement (4.0%), and for dressings (9.7%) in the HHN service content analysis. Conclusion: The progress towards an aging society and the introduction of community care are expected to further enhance the need for HHN which should be able to provide comprehensive and continuous visiting health care services to the older patients. The results of this study are expected to help doctors solve problems not solved by HHN, reduce unnecessary emergency room or outpatient visits, and readmission, while at the same time contributing to the improvement of patient quality of life through efficient patient health care.

Purpose: This descriptive study aimed to identify the relationships among grit, job satisfaction, and the intention to stay of clinical nurses in Korea. Method: Data collected from a total of 239 experienced nurses in four general hospitals of the Jeolla province. The t-test, ANOVA, and Pearson's Correlation Coefficient were performed using the IBM SPSS/WIN 21. Results: There were positive correlations between the grit and job satisfaction of the nurses (r=.31, p<.001), between their grit and intention to stay (r=.25, p<.001), and between their job satisfaction and intention to stay (r=.49, p<.001). Conclusion: Developing and adapting educational programs to enhance the grit enthusiasm, and patience can contribute to increasing the intention to stay of nurses in hospitals.

Purpose: The purpose of this study was to investigate the wage structure of hospital nurses in Korea and to analyze the factors that determine the level of nurses' wages. Methods: First, a mobile survey on nurses' wages was conducted with a total of 3,742 nurses working in hospitals. A literature review was also done. Second, a regression model was applied to analyze the determinants of nurses' wages. Results: The average monthly wage of 3,742 nurses based on the survey was KRW 3,588,000 and the standardized monthly income of 3,742 nurses was estimated to be KRW 3,364,000 as of the end of 2018. The results of regression analysis(R2=61.7%) showed that gender, nursing experience, designated night duty, ownership, and number of beds were statistically significant variables for nurses' wages at 1% significance level. The monthly wage of nurses working in total nursing care wards was not significantly different from those working in other wards at 5% significance level. Conclusion: The substantial difference in nurses' wages according to job career, hospital size, region, etc may result in the low proportion of nurses with more than three to five years of service experience and the high turnover of less-experienced workers. Consideration should be given to adjust the gap in wage level and a standard nurse wage system could be effective measures.

Purpose: The purpose of this study was to examine nurses' knowledge, health beliefs, and performance regarding the infection control of catheter-associated urinary tract infection (CAUTI) and to identify factors affecting their infection control performance of the CAUTI. Methods: The subjects were 166 nurses at three hospitals with less than 300 beds in urban areas.Data were collected using structured questionnaires about knowledge, health beliefs, and performance regarding the infection control of the CAUTI. Statistical analysis included t-test, ANOVA, Pearson's Correlation Coefficients, Multiple regression analysis. Results: The factors affecting the infection control performance of CAUTI were knowledge (β=.18, p=.010), perceived seriousness (β=.25, p=.001), perceived barriers (β=.41, p<.001), and cues to action (β=.15, p=.030), and these factors explained 28.7% of the variance for the infection control performance of the CAUTI. Conclusion: In this study, higher levels of knowledge, perceived seriousness and, cues to action, and lower perceived barriers resulted in a corresponding higher performance in the infection control of CAUTI. Based on these findings, providing intervention programs enhancing the health beliefs of nurses is necessary to ensure their infection control performance of the CAUTI.

Purpose: The purpose of this study were to identify the influential factors of bullying of intensive care nurses and to suggest a final structural model based on identified relationships between nursing organizational culture, authentic leadership, self-esteem, and bullying in the workplace. Methods: Data were collected from 221 nurses at intensive care units in eight general hospitals using structured questionnaires and analyzed by structural equation modeling. Results: In this study, the average of bullying in the workplace was 1.34±0.40, nursing organizational culture was 3.31±0.47, self-esteem was 2.79±0.44, and authentic leadership was 3.61±0.60. The factors affecting nursing organizational culture were authentic leadership (β=.54, p<.001) and self-esteem (β=.24, p=.002) that had direct positive effects on the nursing organizational culture. The nursing organizational culture had also a direct effect on reducing workplace bullying (β=-.45, p<.001). Authentic leadership (β=-.24, p=.004) and self-esteem (β=-.11, p=.004) had indirect effects on workplace bullying, which was mediated by the nursing organizational culture. Conclusion: To understand and reduce workplace bullying, evaluating a nursing organizational culture should be preceded. Based on the finding of this study, an intervention for increasing authentic leadership and self-esteem of nurses can positively help to create the nursing organizational culture and then reduce workplace bullying.
