1A Systematic Review of Chuna Manual Therapy for Adolescent Idiopathic Scoliosis

This study analyzed randomized controlled trials (RCTs) and case studies investigating Chuna manual therapy and variations of this term, for adolescent idiopathic scoliosis. On June 15th, 2019, 6 online databases were used to retrieve studies. A total of 527 articles were retrieved, and 14 RCTs and 20 case studies were selected for review. Typically, the frequency of Chuna therapy was 1-2 times/week. The most common period of treatment was 12 months in RCTs and 3-6 months in case studies. Cobb's angle was the most frequent evaluation index used (11 RCTs and 20 case studies). In control groups, brace treatment was used in 8 RCTs. In 6 RCTs and 20 case studies, Cobb's angle significantly decreased after Chuna therapy, and in 4 RCTs, Chuna therapy was as effective as brace treatment, with no significant difference between groups. Adverse events were not reported except for minor reactions in only 3 case studies. This review suggested that Chuna therapy for adolescent idiopathic scoliosis was more advantageous than and as effective as brace treatment in most cases, although the risk of bias in 13 RCTs was unclear.

2Characteristics of Warm Acupuncture Reported in Experimental Studies: A Descriptive Narrative Review

Background: This study aimed to investigate the correlation between heat transmission and intervention factors for warm acupuncture (such as features of acupuncture material and moxa, the treatment method, and clinical symptoms).
Methods: Korean, English, Chinese and Japanese databases were analysed. Experimental studies that explored the association between thermal stimulation delivery and warm acupuncture intervention factors were included. The peak temperature, time to reach the peak temperature, and time of the effective stimulus, were set as the major parameters and analysed.
Results: A total of 12 studies were included. Two studies were associated with the acupuncture needle material, 4 studies associated with the moxa mass, 1 study associated with the moxa density, 2 studies associated with the location of ignition, and 1 study associated with treatment environment were reviewed. The reporting quality of the 12 studies was low.
Conclusion: This study provided limited information because of the heterogeneity of materials and parameters depending on each experiment. Further studies should clarify the correlation between heat transmission and intervention factors for warm acupuncture.

3Effects of Snake Venom Pharmacopuncture on a Mouse model of Cerebral Infarction

Background: This study investigated the effects of Vipera lebetina turanica snake venom (SV) on cerebral infarction induced by middle cerebral artery occlusion in mice.
Methods: Following cerebral infarction, SV was injected intravenously or added to BV2 cell culture. Tissue injury was detected using triphenyltetrazolium chloride (TTC) staining, neurological deficit score, NO, ROS, and GSH/GSSG assays, qPCR, Western blot, and cell viability.
Results: Cerebral infarction caused by middle cerebral artery occlusion as observed by TTC staining, showed SV inhibited cell death, reducing the number of brain cells injured due to infarction. SV treatment for cerebral infarction showed a significant decrease in abnormal behavior, as determined by the neurological deficit score. The oxidation and inflammation of the cells that had cerebral infarction caused by middle cerebral artery occlusion (NO assay, ROS, GSH/GSSG assay, and qPCR), showed significant protection by SV. Western blot of brain infarction cells showed the expression of iNOS, COX-2, p-IkB-α, P38, p-JNK, p-ERK to be lower in the SV group. In addition, the expression of IkB increased. BV2 cells were viable when treated with SV at 20 μg/mL or less. Western blot of BV2 cells, treated with 0.625, 1.5, 2.5 μg/mL of SV, showed a significant decrease in the expression of p-IkB-α, p-JNK, iNOS, and COX-2 on BV2 cells induced by LPS.
Conclusion: SV showed anti-inflammatory and anti-oxidant effects against cerebral infarction and inflammation.

4The Safety and Analgesic Effect of Datura Flos Pharmacopuncture in Sprague-Dawley Rats

Background: The aim of this study was to investigate the safety and analgesic effects of Datura Flos pharmacopuncture (DFP).
Methods: The analgesic effects of DFP were assessed using mechanical (hot plate), chemical (formalin test), and thermal (von Frey filament test) pain tests. Forty male Sprague Dawley rats were assigned randomly into DFP (75 mg/kg, 150 mg/kg), lidocaine 0.5%, or normal saline group for treatment on Kl3. Gross pathology, histopathology, biochemistry and hematology were performed.
Results: In the hot plate test, DFP at a high dose (HDDFP; 150 mg/kg) produced a significant analgesic effect, at 10 and 20-minutes post injection (p < 0.01). Low dose DFP (LDDFP; 75 mg/kg) also showed an analgesic effect at 10 minutes post injection (p < 0.01). In the formalin test, HDDFP produced an analgesic effect, for 0-10 and 10-20 minutes (p < 0.01) post treatment, whereas LDDFP showed analgesic effects between 10-20 minutes (p < 0.05). In the von Frey filament test, DF-H produced an analgesic effect, 10 (p < 0.01) and 20 minutes post treatment (p < 0.05). LDDFP showed analgesic effect at 10 minutes (p < 0.05). In the acupuncture response test, HDDFP produced an analgesic effect at 10 minutes post treatment (p < 0.05). DF-H did not cause any anatomical changes to the liver or kidney and there were no abnormalities in biochemistry or hematology.
Conclusion: DF-H was not toxic and provided short term analgesia, suggesting it may be useful in the management of pain.

5Analysis of Traditional Medical Disputes: Data from the Korean Acupuncture and Moxibustion Medicine Society (2013-2017)

Background: The purpose of this study was to analyze the medical dispute cases in Acupuncture and Moxibustion treatment, and present the guidelines of treatment to prevent medical disputes.
Methods: Korean Acupuncture and Moxibustion Medicine Society medical dispute cases from January 2013 to September 2018 were collected and analyzed.
Results: There were 80 cases, which included acupuncture treatment (54 cases), cupping treatment (7 cases), local infection / inflammation (17 cases), and neurological symptoms (13 cases). Analysis of the correlation between types of medical accidents and the treatment methods, showed that local infection and inflammation (12 cases) were the most reported in acupuncture treatment.
Conclusion: This study was performed to analyze the current status of medical disputes in the field of acupuncture and moxibustion, and provide basic data for guidelines to prevent them. Further study preparing for clinical guidelines to prevent medical disputes in specific departments are warranted in the future.

6Treatment of Ramsay Hunt Syndrome Using Korean Medicine including Sinbaro3 Pharmacopuncture: A Case Report

This study was conducted to investigate whether treatment with Korean medicine might help to improve symptoms of patients diagnosed with Ramsay Hunt Syndrome. A patient diagnosed with Ramsay Hunt Syndrome was hospitalized for 15 days from October 4, 2018 to October 18, 2018 and treated using Korean medicine (Sinbaro3 pharmacopuncture, acupuncture, electroacupuncture, and herbal medicine). Therapeutic effects were evaluated using the Numerical Rating Scale (NRS), House-Brackmann grade, Yanagihara's score survey, and Digital Infrared Thermographic Imaging (DITI). After treatment, the Numerical Rating Scale score (6 at admission down to 2 points at discharge), House-Brackmann grade (Grade V down to Grade II), Yanagihara's score (6.0 up to 22.0), and Digital Infrared Thermographic Imaging showed improvement from admission to discharge. Treatment with traditional Korean medicine, may help improve symptoms of Ramsay Hunt Syndrome such as facial nerve paralysis, neuralgia after paraplegia, hearing difficulties, and dizziness.

7Korean Medical Treatment Including Miniscalpel Acupuncture for Patients After Rotator Cuff Tear Surgery: A Report of Two Cases

This study aimed to report the effect of Korean medical treatment including miniscalpel acupuncture on 2 patients who underwen tsurgery for rotator cuff tear. They were treated for almost 4 weeks at the Department of Acupuncture and Moxibustion, Dong-Eui University Korean Medicine Hospital. Visual analog scale (VAS), and range of movement (ROM) were used to evaluate treatment effects. In both patients, shoulder pain and restriction of shoulder joint movement improved after miniscalpel acupuncture treatment. In Case 1, shoulder pain decreased from a VAS score 8 to a VAS score 3, and ROM of the shoulder improved from flexion 100° to 160°, extension 10° to 30°, abduction 90° to 130°, adduction 10° to 40°, internal rotation 10° to 50°, and external rotation 10° to 50°. In Case 2, shoulder pain, which was rated a VAS score 8 at first-visit, disappeared, and ROM of the shoulder recovered to normal range. These results suggest miniscalpel acupuncture may contribute to the recovery process after rotator cuff tear surgery.

8A Patient with Ankylosing Spondylitis Treated with Acupotomy and Traditional Korean Medicine

This case report relates to a 40-year old male patient diagnosed with ankylosing spondylitis who was treated with acupotomy and traditional Korean medicine. He showed a significant improvement in joint range of motion (ROM) in cervical and lumbar vertebrae, and in pain and functional disorder symptoms. The patient received acupotomy, acupuncture, pharmacopuncture, herbal medicine and physical therapy (November 26, 2018 to December 17, 2018). Joint ROM for cervical and lumbar vertebrae was measured, and the pain level was assessed using a numerical rating scale. The functional disorder and quality of life scales were also assessed using the Bath Ankylosing Spondylitis Functional Index, Bath Ankylosing Spondylitis Disease Activity Index, Korean Health Assessment Questionnaire, and Modified Health Assessment Questionnaire. After applying acupotomy and traditional Korean medicine, the patient exhibited increased joint ROM and reduced pain, also in conjunction with improved responses in functional disorder and quality of life.

9A Case of Dural Ectasia with Low Back Pain and Sciatica Treated with Integrative Korean Medical Treatment

Dural ectasia is defined as ballooning or expansion of the dural sac surrounding the spinal cord. This report describes a rare case of low back pain and sciatica, suspected as being dural ectasia. The patient was hospitalized for 45 days, and underwent integrative Korean medical treatment, including pharmacopuncture, acupuncture, herbal medicine, Chuna therapy, cupping therapy, and physiotherapy. The effect of the treatment was evaluated using the numerical rating scale, Oswestry disability index, European quality of life 5 dimensions, and subjective symptoms. After inpatient treatment, the pain the patient experienced was significantly reduced and the evaluation indices improved. This case report suggested that integrative Korean medical treatment could be an effective therapeutic choice for low back pain and sciatica, with dural ectasia. Further clinical studies are needed to support this observation.
