A Survey on the Current Status of Neonatal Physical Therapy in South Korea

Purpose: This study aimed to investigate and report the current status of physical therapy (PT) performed in Korean neonatal intensive care units (NICU) to present foundational data that promotes the advances in neonatal PT in Korea.
Methods: Based on the Health Insurance Review and Assessment (HIRA) data, we administered a questionnaire survey to 74 hospitals (39 tertiary and 35 general hospitals) in Korea equipped with a NICU and pediatric PT unit. We developed a 32-item questionnaire with reference to previous Korean studies. The questionnaires were distributed and retrieved via regular mail and an online system.
Results: Of the 74 hospitals, 58 (78%) practiced neonatal PT and the duration of each session significantly differed according to the hospital rating. PT was given, depending on clinical symptoms, to infants who were preterm and low birth-weight (96.5%), had brain and spinal cord diseases (84.5%), had pathological tonus (94.8%), with respiratory problems (65.5%), for range of motion exercises (82.8%), for neurodevelopment approaches (72.4%), and for positioning (70.7%). Interdisciplinary meetings were held to share clinical decision-making in 17.2% of the hospitals surveyed and parent-participating education to ensure a family-centered approach was offered in 63.8% of the hospitals. The barriers of neonatal PT included low insurance fees, insufficient awareness of colleagues, and the severity of the patient.
Conclusion: This study is the first report of the current status of neonatal PT in Korea. The findings of this study will serve as foundational data to review the current neonatal PT practice and promote further advances.

1Effects of Various Types of Bridge Exercise on the Walking Ability of Stroke Patients

Purpose: The purpose of this study is to examine the effect of various bridge exercises on walking ability.
Method: The subjects were 30 stroke patients. They were divided into a bridge exercise group on a stable support surface (Group I), a bridge exercise group on an unstable support surface (Group II), and a bridge exercise group combined with whole body vibrations (Group III). 10 subjects were randomly assigned into each group. The subjects of this study had 30 minutes of nervous system physical therapy including gait training and strength training. In addition, each group underwent a 30 minutes session five times a week for eight weeks. Before intervention, LUKOtronic was used to measure step width and step length, time was measured with a 10 m walking test, and time and number of steps were measured with the figure 8 walking test. After the intervention, remeasured and analysis was performed for each group.
Results: As a result of comparing and analyzing the change of walking ability between groups, there was a statistically significant difference. As a result of the post hoc analysis according to the change of walking ability among groups, the change of walking ability was larger in Group III than in Group I and Group II.
Conclusion: Based on these results, it is confirmed that the bridge exercise combined with whole body vibration was more effective for walking ability. Based on these findings, this study proposes an effective program for elite athletes as well as stroke patients.

2The Effect of Breathing Training on the Physical Function and Psychological Problems in Patients with Chronic Stroke

Purpose: To identify the effect of breathing training on the physical function and psychological problems in patients with chronic stroke.
Methods: In total, 26 patients with chronic stroke were randomly assigned to one of two groups: the experimental group, who underwent breathing training and neurodevelopmental treatment, and the control group, who underwent neurodevelopmental treatment (13 patients per group). Physical function was evaluated using the balance and Activities of Daily Living (ADL). Psychological problems were assessed using the Depression. Balance was measured using the Berg Balance Scale (BBS). ADLs were measured using the Modified Barthel Index (MBI). Depression was measured using the Beck Depression Inventory (BDI). The BBS, MBI, and BDI were recorded before and after the 4-week treatment.
Results: As a result of comparison within groups, the experimental and control group showed significant difference for BBS, MBI and BDI after the experiment (p<0.05). In comparison between two groups, the experimental group showed more significant difference in BBS, MBI and BDI than the control group (p<0.05).
Conclusion: This study showed that breathing training was effective improvement physical function, and psychological problems in patients with chronic stroke.

3Comparison of the Static Balance Ability according to the Subjective Visual Vertical in Healthy Adults

Purpose: The subjective visual vertical (SVV) test is used to evaluate the otolith function in the inner ear. This study compared the different balance ability according to the results of the SVV in healthy adults.
Methods: This study recruited 30 normal healthy subjects who did not have neurological and musculoskeletal disorders. The subjects were divided into experimental and control groups based on the results of SVV: experimental group, >2°; control group, <2°. The static balance ability was evaluated using the Fourier Index, which could evaluate the balance capacity objectively and quantitatively.
Results: The mean angle of the SVV in the experimental and control groups was 4.44° and 0.59°, respectively. In the result of the Fourier series, the F1 frequency band in the experimental group showed a significantly higher value under one condition compared to the control group (p<0.05). In the F2-4 and F5-6 frequency bands, the experimental group showed a significant increase in the Fourier series value under the four conditions compared to the control group (p<0.05). In the F7-8 frequency band, significantly higher values of the Fourier series were observed in the experimental group under the three different conditions (p<0.05).
Conclusion: These results showed increased trunk sway while maintaining static balance in the experimental group who showed a larger SVV angle compared to the control group. The SVV can be applied to evaluate the vestibular system and balance ability in normal adults.

4Effect of Shoulder Position on Scapular Muscle Activity during Scapular Protraction

Purpose: This study was to evaluate several tasks performed at a high intensity in terms of their ability to elicit EMG activity in the serratus anterior by comparing the EMG activities of the serratus anterior, upper trapezius, and lower trapezius muscles during six tasks combined shoulder flexion with rotation.
Methods: Fifteen healthy males were recruited to this study. Each subject was instructed to assume a sitting position without back support and asked to flex (90° or 120°) the right shoulder and protract the scapula in the sagittal plane with maximal external rotation; to assume a neutral position; or to internally rotate the glenohumeral joint. The EMG data were collected from the serratus anterior (SA), upper trapezius (UT), and lower trapezius (LT) muscles were normalized to maximum voluntary isometric contraction. The UT/LT and UT/SA muscle activity ratios in each task were assessed by calculating the surface EMG. Data were analyzed by two-way repeated-measures analysis of variance, with the level of significance set at p<0.05.
Results: The results of this study, shoulder flexion with external rotation resulted in low upper trapezius/serratus anterior and upper trapezius/lower trapezius ratios and a relatively high level of serratus anterior activation.
Conclusion: Shoulder flexion with external rotation used herein may be considered as important for clinical interventions aimed at selectively increasing SA strengthen and clinical selection of exercises for improving glenohumeral joint and scapulothoracic control.

5Cross-education Effects of Muscle Strength and Balance on Unilateral Isokinetic Exercise in Ankle

Purpose: Unilateral strength training effects on contralateral sides have been demonstrated in previous studies for lower extremity exercise, upper extremity exercise, and unilateral surface electrical stimulation. This study was performed to investigate the effects of unilateral ankle training on muscle strength and the balance of contralateral lower extremity in healthy adults.
Methods: Thirty healthy subjects were randomized equally to a training or a control group. Those in the training group received unilateral ankle isokinetic strengthening training of the dominant leg (right side) for 4 weeks. Contralateral single-limb balance, including Anterio-Posterior Stability Index (APSI), Medio-Lateral Stability Index (MLSI) and Overall Stability Index (OSI), was assessed before and after intervention.
Results: Comparison of pre- and post-test data revealed significant improvements in ipsi- and contralateral ankle strengths, and significant improvement in contralateral single limb balance.
Conclusion: These results have practical implications because they demonstrate that unilateral ankle isokinetic exercise improves ankle muscle strength and balance ability of contralateral lower extremity.

7Changes in Grip Strength and Associations with Grip Strength in Breast Cancer Survivors Treated with Adjuvant Chemotherapy

Purpose: The aim of this study was to identify the change in hand grip strength (HGS) for consecutive 6 months in breast cancer survivors (BCS) undergoing postoperative adjuvant chemotherapy (adjCTX) and to determine the factors relevant to HGS after chemotherapy completion
Methods: This study was a retrospective observational cohort study. BCS (N=38) who underwent breast cancer surgery and subsequent adjCTX were enrolled. The HGS of both sides was measured before adjCTX and 3 weeks, 2 months and 6 months after adjCTX. And we also collected body composition, anthropometric measurements, social demographics and clinical data. Then we analyzed the changes in HGS over time, and correlation of collected data with HGS at 6 months.
Results: A significant increase in HGS of the unaffected side was observed within the first 3 weeks of adjCTX and maintained up to 6 months. In correlation analysis, HGS of both sides was positively correlated with the ipsilateral HGS at 3 weeks, and contralateral HGS at 6 months. And skeletal muscle mass was also positively correlated with the HGS of the unaffected side. In addition, subjects treated with targeted therapy had lower HGS of the unaffected side. However, HGS of the affected side was significantly lower in subjects with breast reconstruction and treated without radiotherapy.
Conclusions: To maintain the early recovery of HGS, active and continuous exercise intervention for strengthening might be necessary.

8Virtual Reality Community Gait Training Using a 360° Image Improves Gait Ability in Chronic Stroke Patients

Purpose: Gait and cognitive impairment in stroke patients exacerbate fall risk and mobility difficulties during multi-task walking. Virtual reality can provide interesting and challenging training in a community setting. This study evaluated the effect of community-based virtual reality gait training (VRGT) using a 360-degree image on the gait ability of chronic stroke patients.
Methods: Forty-five chronic stroke patients who were admitted to a rehabilitation hospital participated in this study. Patients meeting the selection criteria were randomly divided into a VRGT group (n=23) and a control group (n=22). Both these groups received general rehabilitation. The VRGT group was evaluated using a 360-degree image that was recorded for 50 minutes a day, 5 days per week for a total of 6 weeks after their training. The control group received general treadmill training for the same amount of time as that of the VRGT group. The improvement in the spatiotemporal parameters of gait was evaluated using a gait analyzer system before and after training.
Results: The spatiotemporal gait parameters showed significant improvements in both groups compare with the baseline measurements (p<0.05), and the VRGT group showed more improvement than the control group (p<0.05).
Conclusion: Community-based VRGT has been shown to improve the walking ability of chronic stroke patients and is expected to be used in rehabilitation of stroke patients in the future.
